We have adopted a new system for completing forms, having transitioned most of the required information into one online Google registration form. Below, you’ll see two categories of forms: ones that are to be completed online, and ones that need to be printed and turned in.
ONLINE FORMS – Complete online and submit.
- Google Registration Form (Read the following instructions first.)
- This form will ask you to verify that you have read and understand a number of documents. Please read them prior to starting the form. No need to print any of these, as they are for reference only.
- They include:
- US Rowing Waiver (Varsity rowers, complete before your previous waiver expires. Novice rowers, complete this after we notify you that your profile is set up and ready to access.)
- Norfolk Parks and Recreation Swimming Pool Registration (After you complete this, email it to granbycrew@gmail.com.)
- NPS Volunteer Form
PAPER FORMS – Print, complete and submit to the noted person.
- VHSL Sports Physical (Submit to financial secretary.)
- NPS Cardiac Arrest and Concussion Forms (Submit to financial secretary.)
- NPS Volunteer Driver Form (Submit to financial secretary.)
- Financial Assistance Application (Submit this to the financial secretary only if you have not already completed the financial assistance portion of the Google Registration Form.)
